VFRT Case Report Form
Instructions: Use this form to submit one case report for each of the 10 Training Modules starting with Hand and Wrist.
You do not need to do the case reports in order, and you can combine reports if you worked on several conditions with one client at a time. Just note in the report that you are writing about more than one Module.
The client you work on must be an adult (over age of 18 yrs old), a human, and not you. Use your judgement on what you work on. If you are uncomfortable working in locations on the body that we teach in the course, skip this area especially if there is a conflict with one of your existing modalities.
You can even work on someone who doesn’t have any of the conditions in the module. Just work on the area and palpate just like the videos. Experience the tissue density and note the quality of every anatomical structure.
Return back to this form and fill it out for each report. I will review each one and provide feedback and comments if there is something worth reporting. Do not wait for feedback before starting on the next one. You can submit all 10 reports at once if you have completed the sessions.
